The original route of the B4158 ran through Wolverhampton city centre, along Garrick Street, Market Street and Princess Street. It became part of the A459 upon the pedestrianisation of Dudley Street in the late 1960s before being downgraded entirely in 1986 when the A4150 Inner Ring Road was completed to the east of the city centre.
It is now impossible to traverse the entire route in either direction using a car. Northbound traffic may use Garrick Street, but then a short stretch between Bilston Street and Tower Street is restricted to buses. North of Tower Street, the use of the road is unrestricted, but clever use of turn restrictions from side roads effectively bans the majority of cars on the remainder of the route, though U-turning (whilst not recommended) is permitted, as is turning from very minor streets. Southbound traffic may proceed south from Princes Square along Princess Street and Market Street, but a short section immediately south of Bilston Street is northbound only, though traffic (subject to restrictions) may turn south out of Old Hall Street.
