Tollbar End during the improvement works to add an underpass. This picture was taken from a nearby unclassified road (shown at the bottom-right of the picture), and shows the exits to the B4110 (at the right of the picture), the A45 from/towards Birmingham (at the top of the picture), and the A46 (at the bottom-left). The roundabout itself is also visible (but a relatively small part of the picture), and is in the top left (where the black and white chevron sign is).
At this stage in the improvements, the A45 westbound approaching the junction had been given a new alignment in which going straight on goes directly onto the A46 (which is clearly visible), and turning right to some extent is needed to go onto the roundabout (making this a TOTSO arrangement; from this angle, the roundabout looks almost like a crossroads). The main moving flow of traffic at the instant the photo was taken was from the roundabout onto the A46 (because most other flows were stopped by traffic signals). The centre-right lane (with the car) is a filter lane from the B4110 onto the A46, with the white lorry on the path from the B4110 onto the roundabout itself.
