Scour beneath the abutment. Foundations either not deep enough or not integral with the abutment. It was always a bit of a concern to me when I had responsibility for large older bridges over fast flowing rivers that we had no records of foundation depths or indeed sometimes how the abutments tied into the foundations. One precautionary measure we used was to sheet pile around the abutment (water side, and with returns into the bank) and backfill with concrete. This could all be done beneath normal water level and thereby not impinge on the bridge appearance except when the river was unusually low.
