The B4209 is a largely rural B-road within sight of the Malvern Hills.
It starts on the A449 in Malvern Wells and follows Hanley Road. Initially it heads southeast to reach Green Lane, which starts on the A449 just to the south of Hanley Road, before taking that road's course due east.
After passing the Three Counties Showground the road crosses the B4208 at traffic lights. A mile further on it reaches Hanley Swan, where it passes the village green and duck pond. The road now starts to bend to the right and even zigzags for a short distance.
A stream is crossed before the B4209 enters Hanley Castle. It ends at a T-junction on the B4211 (former A440) at the northern end of the village. The castle, of which very little remains, was at the other end of the village.
