The road originally started in Port Talbot docks and ran along Newbridge Road and Victoria Road into Aberavon. Most of this section was later declassified, although the route was instead extended westward along the dual-carriageway Afan Way to reach the M4's temporary terminus at Baglan (now part of junction 41). Eventually this led to a gap in the B4286, separated by some 430 m of the A48 Heilbronn Way viaduct near the town centre - but this is itself a replacement for the original line of the B4286 which ran adjacent to the river and is now lost under the Aberafan Centre.
Afan Way has since been upgraded to become the A4241, so the B4286 now starts at the A48 Pentyla Baglan Road / Heilbronn Way roundabout by Port Talbot Civic Centre. It heads east for 150 m or so before turning left to pass under the M4 and begin its ascent of the Afan valley. (Traffic coming down the valley and heading for the A48 is obliged to make a left turn at this junction, briefly cross the river, and double back at the next roundabout.)
Following the right (western) bank of the River Afan (on a route paralleling the A4107 on the other side of the valley), the B4286 runs through Cwmafan before eventually passing beneath an impressive (disused) railway viaduct in Pontrhydyfen and terminating at a T-junction with the B4287.
