The road starts at the Holme Lacy Bridge crossing of the River Wye and zigzags westwards to Ross Road on the south side of Hereford. It also forms part of an effective southern Hereford bypass, linking the aforementioned M50–Hereford route to the A49 via the Rotherwas Access Road, although the narrowness of the B4399 and a series of tortuous bends west of Holme Lacy make this a less than suitable route for long or heavy vehicles. Prior to the construction of the Rotherwas Access Road the B4399 followed a straighter route through Putson.
