The C165 forms a loop route starting on the C164 at Tarlaraghan. From here the road heads east before sharply turning north east, leading to the A4. The road continues from the opposite side of the A4, continuing north east as it passes over the M1. Almost immediately afterwards, the road crosses the C161 at a scattered crossroads. From here the road heads north and loops round to the west as it approaches the River Bann. Once more the road swings round to the north to terminate on the C160 a short distance west of where the Upper Bann flows into Lough Neagh.
