The C21 maintained by Kent Council is a Class III road that runs in a north-south direction.
Continuing from Medway's C21, the C21 heads in a southerly direction away from Rochester. Passing from farmland and following the river Medway, the C21 enters the village of Wouldham as the High Street. Leaving the village, the road takes a sharp turn east, onto Knole Road, avoiding the large Peters Village development. Turning south and becoming Rochester Road again, the C21 passes through Burham. Shortly after, the road TOTSOs west onto Bull Lane - the continuing Pilgrims Way leads to the A229.
Continuing in a roughly southerly direction, the C21 enters the village of Eccles before leaving and passing through farmland and between two reservoirs. At the entrance to Aylesford Priory, the road takes a sharp eastern turn to become High Street and enter Aylesford. Passing the historic Station Road bridge over the river Medway the C21 has a junction with the C25 before terminating on the C349 in the centre of the town.
