The C221 starts on the A25 east of Belleek and heads in a southward direction climbing onto higher ground. The road crosses the B30 on its way through this somewhat hilly landscape. Eventually the village of Mallaghbane is reached from which the C226 heads off to the west. A spur leads north from the village back to the B30 at Ballykeel, with the C226 turning off of this spur to the east. Continuing south east, the road reaches Forkhill where it encounters the C224 on the west side of Kilcurry River. The road crosses the river and ends on the B134 on the opposite side.
