The C423 is a rather complex route, totalling over 18km in length. It starts on the westbound only A4 Main Street of Fivemiletown and follows Altaveedan Road south east into the countryside. Before long a crossroads is reached and from here the route continues both north east and south. To the north east, the road makes its way back to the A4 near Ballagh Lough. En route a spur swings round to the south east and runs along the northern slopes of the extensive upland landscape surrounding Slieve Beagh to the south. Soon this spur turns north as it too meets the A4, this time at Findermore. The route south from the crossroads makes its way through some rather quiet and forested countryside for a few kilometres. The road eventually crosses the Colebrooke River via Glennoo Bridge and ends on the nearby T junction to the south.
