The C424 is a very long route primarily within County Fermanagh, but with a short segment within County Tyrone. Starting on the A4 in Fivemiletown, County Fermanagh, the road heads south and soon meets the C451 at a TOTSO. Turning left, the road enters County Fermanagh and soon resumes its southward journey. The road crosses the Colebrooke River via Pollboy Bridge on its way to The Cross, where it briefly multiplexes with the C425. The road winds its way across the moorlands before encountering the C427. After this junction the road turns south east and descends down off of the moors, meeting the opposite end of the C451 as it does so. Finally, the road comes to an end upon reaching the B36 at Dernawilt Cross Roads.
