The C451 consists of two very separate routes. The longer southern route is almost entirely within County Fermanagh (saving its north end which lies along the Fermanagh-Tyrone border) and starts at a TOTSO on the C424 south of Fivemiletown and makes its way south. The road ascends into some rather quiet upland landscape where it eventually meets the C425 at a crossroads. Further south, the road drops back down towards farmland and ends at a T junction on the C424. The shorter northern route starts in County Tyrone on the B72 at Shanvia and heads south west, very soon entering County Fermanagh. This stretch of the C451 is very straight unlike its southern component at Fivemiletown. The road ends at a crossroads with the C450 continuing west and the C403 continuing south and east from here.
