The C619 is a rather complicated route in east County Tyrone. It starts on the B520 south of Cookstown and heads south east towards a scattered crossroads. Here the C619 splits into two separate roads with one continuing east from here and the other heading south east. The road south east is quite straight and meets the B160 north of Stewartstown. The road to the east begins to turn in more of a south easterly direction before it too meets the B160. Just across the road from here the route continues and soon TOTSOs east. At the next crossroads the road continues in all three other directions. To the south west the road returns to the B160 north of Stewartstown, closer to the village than spur on the other side of the B160. To the north east the road connects to the C564 and to the south east the road reaches the B161 near Killycolpy.
