The C621 starts on the B4 at Pomeroy and heads north across the Tyrone countryside for several kilometres. As it does so it crosses the routes of two separate segments of the C624 at different crossroads. The road gradually swings north west before sharply turning north to meet the A505 at a scattered crossroads. The road continues north and soon meets one of many legs of the C612 at a T junction, continuing from another junction not far to the west. The final stretch of the road takes the C621 over the Owenkillew River and heading north westwards it comes to an end on the C554.
