The C622 forms a V shaped route to the south west of Cookstown. It starts at Loughry Roundabout on the A29 in the south of Cookstown. From here the road initially heads west, but soon turns smoothly to the south west with a spur heading north from the bend to meet the A505. After the bend the road is generally rather straight as it heads towards Sandiholes. At Sandiholes the road meets the C623 while to the north the remainder of the route turns off to the north west. This leads the route to the B4 where it resumes from a junction just to the east. The road soon comes to an end on the A505 east of Drum Manor Forest Park.
