The C649 starts on the old alignment of the A4 - the B34, and heads south where it soon passes over the modern dual carriageway A4. The road continues south west and crosses Oona Water before coming to a crossroads. At the crossroads the road is crossed by another component of the C649 which runs from the B35 in the south east back to the B34 in the north west, after passing underneath the A4. To the south of the crossroads the road is generally quite straight as it reaches the B35 at Castletown. The C649 also includes a spur which links to the B35 opposite its junction with the western spur of the C646, passing around Mullaghbane Moss.
