The C658 overall forms an X shaped route spanning the countryside between Clogher and Fintona, consisting of two clearly distinct components meeting at a crossroads in the middle. The longer of the two components starts in the south west on the B122 and heads north east uphill towards the shoulder of Ballyness Mountain, reaching an altitude of over 250 metres. Descending down the other side, the road soon comes to the crossroads where it meets the other component of the C658. Continuing north, the road crosses the B168 and swings round to the east to meet the B83. East of the B83, the road comes to a T junction and TOTSOs north, soon ending on the C657. The other component of the C658 provides a link from the B168 in the south east to the B122 south of Fintona in the north west.
