The C681 is a rather complicated route in the south west Tyrone. It starts at a tight bend on the B84 and heads south towards the A32, crossing the C405 as it does so. South of the A32 the road crosses the C683 and before long a spur returns to the A32 along Moorfield Road. South of this spur a crossroads is reached and from it the route continues east and west. To the east the road makes its way to the village of Trillick, where it meets the B46. The B46 is also met taking the western segment from the crossroads as it turns south westwards. Along this stretch of road a further spur turns off to the north west and soon TOTSOs west, leading eventually to the A32 outside Irvinestown. The two segments of the C681 leading to the B46 from the crossroads are linked by another section of the C681 which follows Scallen Road. Along the segment to the west, the road continues beyond the B46 and eventually reaches the C413 at a junction very near the Fermanagh/Tyrone border.
