The C78 starts on the B6360 below the Leaderfoot Bridge and quickly crosses the Leader Water as it heads east. At the top of the hill, there is a short multiplex with the B6356, which doglegs across the C78 route. Continuing east, the route follows the contours below Redpath and Brotherstone Hills, before dropping a little to reach the B6397 at Smailholm.
