As a starter, this one on the A627(M) has 5 but I'm sure it can easily be beaten.

Eight. But alas no more, the replacement is a mere six, with a few places edging slightly closer, and they're not bothering with brackets either

In Northern Ireland, Co. Derry seems to specialise in these:
Six destinations on the A29 Coleraine Ring Road. Only Garvagh and Cookstown are actually on the A29.
Six destinations on the A6 east of Dungiven. Randalstown and Castledawson seem unnecessary. Larne isn't even on the A6 but also is signed from Derry city centre.
Five destinations on the A29 south of Garvagh. Swatragh is the next village en route but Tobermore seems unnecessary. At least all these are actually on the A29.
