I was just thinking of the lights at rising bollards. They have only amber and red aspects - going to flashing amber (accompanied by a warning sound) while the bollards are about to rise and then red when they are up.

Here in Ireland, even two-aspect pedestrian lights are in fact not that common nowadays, mostly existing only at pelican crossings.
