The C4 starts on the A73, West Port, at the west end of Lanark, and heads north. It soon drops steeply down to cross the Mouse Water before an equally steep re-ascent. Before long it reaches the railway, but the old level crossing has been closed up, forcing a lengthy diversion to the east and back through a pre-existing overbridge. Back on the original line, the route continues north to reach the A721 at Kilcadzow.
