The B4205 is an urban B-road which zigzags its way through Worcester city centre.

Starting on the A44 gyratory at Dolday, it heads north and then north-east along The Butts, Shaw Street, and Sansome Street to meet the A38 first at The Foregate (northbound A38) and again by St George's Catholic church (southbound A38). All of this section is one-way from Dolday.

A diagonal crossing of the southbound A38 leads into what was the original course of the B4205 along Lowesmoor, but as access along this route is restricted (limited between 15.30 and 18.30 to buses and cycles only), the B4205 is now interrupted at this point and resumes further along the A38 at the City Walls roundabout, from where it heads east along St Martin's Gate then north along Pheasant Street before rejoining Lowesmoor on the approach to a mini-roundabout junction with the B4550.

After crossing the Worcester & Birmingham Canal the B4205 veers to the right again at traffic lights to follow Shrub Hill Road. Worcester Shrub Hill station was the original (1922) terminus of the route but now it continues south along Midland Road to reach a mini-roundabout on Wyld's Lane. The B4205 turns left here, to follow the eastern arm of this crescent-shaped road before terminating at traffic lights on the A44 London Road.
