The original line of the B4482 in Worcester was parallel to the A38, meeting it at both ends. It started in the city centre, just to the east of the Cathedral, and headed north up Friar Street and New Street to the Corn Market. A dogleg through the square led to Queen Street, and then across the B4205. The two routes very briefly multiplexed eastwards, before the B4482 wound from Sansome Street into Sansome Place and then Sansome Walk in quick succession. Kinking right around the Grammar School, the route then forked left into Tennis Walk and Little London before turning sharp left back to the A38.
The middle section of the route, including much of Sansome Walk, is now part of the southbound A38 itself.
