Despite being nearly three miles long, the B6199 is rather straightforward. It takes an east-west route in practically a straight line to the south of Bolton town centre.
The road starts at a signal-controlled junction with the A575 and heads west along the bottom end of Bolton, slicing its way through the Harper Green and making a few stops for mini-roundabouts before coming to a crossroads. This crossroads is signal-controlled and the B6199 meets an unclassified route that takes traffic into Bolton. After a short distance, the road meets another set of traffic lights for Bolton Hospital; the road continues its westward trek, only encountering another mini-roundabout and a set of traffic lights (for the unclassified link road down to M61 J4) before it meets its end at a Give Way sign at the A579.
