The B45 is one of a number of routes which connect the A5 and A7 along the south coast of the Isle of Man. It starts on the A5 Coast road at Rhenwyllan, the straggling eastern extremity of Port St Mary and heads north west between houses on Mount Gawne Road. It then kinks hard right and heads out across the fields, with a line of modern bungalows along the left hand side of the road. The route then dips past Moorhouse Farm to cross the Isle of Man Steam Railway at a level crossing, before climbing a little up to end on the A7 at a simple T junction a short distance to the east of Ballagawne Roundabout
